Look at the Tintaya Mine case: HBSP No. 9-506-023. (B case: 9-507-030) Also, check out the Tintaya Mine website to see what has happened since the case set was written:

They have a section on their website telling about their sustainable operation program.
I just used the case in an MBA Social Responsibility course at a South American university, and it worked well. It provided a good basis for teaching stakeholder theory and doing an exercise in stakeholder analysis. It also does a good job of portraying the role of international NGO's as facilitators.

Dear BPS-netters:
I'm looking for a syllabus for a MBA level course to teach "Sustainability and Business".
By 'sustainability', I mean more than meeting the shareholder's expectation relentlessly and generating economic outcomes continuously. I would like to address the environmental and social concerns in business.
