Thanks to all who responded to my earlier request for information. The list of possible alternative online survey tools to Survey Monkey:

    www.qualtrics.com

    www.zoomerang.com/

    www.polldaddy.com/

    Google Docs (http://www.ehow.com/how_4615530_survey-using-google-documents-free.html)

    www.surveygizmo.com/

    www.questionpro.com

    www.encuestafacil.com

http://limesurvey.org/ (open source, so it is (1) free, (2) changeable, and (3) has a large, helpful user community, but you will need your own webspace (i.e., a server with PHP and MySQL).

Of these, Qualtrics was the tool which received the most favorable comments.

Sorry for the delay in my response.

I have conducted surveys of top execs, and used my university URL for that. My impression is that a generic survey site will cause suspicion among executives, especially when sharing sensitive information. Having a .edu URL will provide more credibility. Of course, this means that your own IT department will have to implement the survey, for better or worse. My advice is to submit weekly test answers yourself, to make sure all is working properly.
